I'd say Kaka and Gerrard personally but Riquelme, Deco, Nedved, and Fabregas are up there as well.
So for the poll my vote goes to Gerrard.

Pirlo is an excellent midfielder. De Rossi will be up there one of these days as well. I'm kind of drawing some blanks here when it comes to midfielders. When I can think of some more guys I'll definately comment again.
EDIT: I do agree that Deco is a great midfielder, but you also have to look at guys like Zambrotta and Camoranesi. Zambrotta might be played as a full-back, but he is a midfielder as well and started out that way.

I don't feel that Camoranesi is really comparable to the likes of players such as Gerrard and Kaka' but he
is Argentinian so I'll go ahead and concede that argument anyways.

He's is an experienced player that is proven so I threw him in the list. I was having trouble thinking of good midfielders that are still currently playing so I thought it sounded good putting him there

You might also consider Totti a striker because he is often played in the midfield behind two strikers.

Steven Gerrard for me is the best midfielder in the world at present. The guy doesn't really have a weakness, you could play him in any outfield posistion and he would do a job for you as good or better than the player who naturally plays that posistion.
Gerrard has played Defensive Midfield, Attacking Midfield, Right Midfield and Right Back in his time for Liverpool and he has been our best player on the field in any of those posistions, he sometimes switches wings and plays Left for a few minutes here and there and always makes something happen.
This guy is special, he can do things that alot can't. He grabs the game by the scruff of the neck when Liverpool are down and he brings us back to life, he is an inspirational captain and leads by example. It would be easy here to mention the Champions League game inspiring us back to 3-0 with some help from Hamman, and the same in the FA Cup Final last season bringing us back from 2-0 down and scoring a 35 yard half volley in the 91st minute when we were down 3-2.
He just has the nack of scoring important goals for us, Olympiakos is another example in the 88th minute to send us through into the CL knockout stage which we went on to eventually win.
He is the second best passer in the league behind Xabi Alonso, his stamina is matched by few, his blend of attacking and defensive skills again is rare in this day and age and his gritty and classy performances have made him become one of the greatest players to of ever played for Liverpool and hes still only just 26.
If Gerrard retired now he would be a Liverpool Legend, he has won EVERYTHING with the club that is possible apart from the Premiership which i am highly confident he will do before he retires to give him a nice set of Carling Cup(s) FA Cup(s) Uefa Cup, Champions League, Charity Sheild(s), Super Cup(s), etc.
Nobody matches Gerrard at the minute in Midfield.

Its a close one between Kaka and Gerrard. How Cole, Lampard are on that list ill never know.
In 2 seasons time i can confidently assure Gerrard will be 1 of the all time greats of this game.

Some people can consider Dinho as a forward, but he is also a midfielder, so , for me (if he is trully the world best player) he should be the best midfielder.
There are also some great midfielders that wern't included in the list, like Essien, Gattuso, Vieira, Mascherano, Diarra, Makelele, Frings, etc. but they are defensive midfielder, but midfielders as well!
Choosing the best playern in a position is always hard, there are manu great of them to pick, but if i would have to choose only one, it would be definitevely Steven Gerrard.
